Aetiology of Urethral Strictures at Moi Teaching and Refferal Hospital
IntroductionUrethral stricture disease is still a major cause of morbidity and life-long disability in third world countries. We reviewed the records of patients at MTRH to determine the aetiology of urethral strictures.Methods and MaterialsA retrospective review of patients’ records from 2003 to 2007, was carried out. Variables of interest included age, gender, and aetiology of urethral strictures.Results There were 143 patients out of which 112 had complete data that could be analyzed. Fifty one percent of urethral strictures were due to urethritis, 47% were due to trauma and 1.8% due to rare causes like urethral diverticulum and urethral carcinoma. Accidental trauma caused urethral strictures among younger people 27.7+/-14.3 years while strictures due to urethritis were commoner among middle aged patients 45.6+/- 16.3 years.ConclusionUrethritis, iatrogenic and accidental trauma are still the commonest causes of urethral strictures in our environment. Functional Cellular Anti-Tumor Mechanisms are Augmented by Genetic Proteoglycan Targeting.
While recent research points to the importance of glycans in cancer immunity, knowledge on functional mechanisms is lacking. In lung carcinoma among other tumors, anti-tumor immunity is suppressed; and while some recent therapies boost T-cell mediated immunity by targeting immune-checkpoint pathways, robust responses are uncommon. Augmenting tumor antigen-specific immune responses by endogenous dendritic cells (DCs) is appealing from a specificity standpoint, but challenging. Here, we show that restricting a heparan sulfate (HS) loss-of-function mutation in the HS sulfating enzyme Ndst1 to predominantly conventional DCs (Ndst1f/f CD11cCre+ mutation) results in marked inhibition of Lewis lung carcinoma growth along with increased tumor-associated CD8+ T cells. In mice deficient in a major DC HS proteoglycan (syndecan-4), splenic CD8+ T cells showed increased anti-tumor cytotoxic responses relative to controls. Studies examining Ndst1f/f CD11cCre + mutants revealed that mutation was associated with an increase in anti-tumor cytolysis using either splenic CD8+ T cells or tumor-infiltrating (TIL) CD8+ T cells purified ex-vivo, and tested in pooled effector-to-target cytolytic assays against tumor cells from respective animals. On glycan compositional analysis, HS purified from Ndst1f/f CD11cCre + mutant DCs had reduced overall sulfation, including reduced sulfation of a tri-sulfated disaccharide species that was intriguingly abundant on wildtype DC HS. Interestingly, antigen presentation in the context of major histocompatibility complex class-I (MHC-I) was enhanced in mutant DCs, with more striking effects in the setting of HS under-sulfation, pointing to a likely regulatory role by sulfated glycans at the antigen/MHC-I - T-cell interface; and possibly future opportunities to improve antigen-specific T cell responses by immunologic targeting of HS proteoglycans in cancer
Quantitative analysis of gallstones in Libyan patients
Gallstone disease is one of the major surgical problems in the Libyan population; it is probably related to diet, especially excessive consumption of meat. The study was conducted to determine the composition of gallstones and their possible etiology in a Libyan population. The chemical composition of gallstones from 41 patients (six males and 35 females) was analyzed. The stones were classified into cholesterol, pigment, and mixed stones (MS). Cholesterol stones (CS) showed a significantly higher cholesterol content than pigment stones (PS) (p=0.0085) though not significantly higher than MS. Their phospholipid content and inorganic phosphates were higher than in the other types of stones and oxalate content was significantly elevated in comparison with MS (p=0.0471). In MS, the cholesterol, bile acids, and bilirubin were intermediate between cholesterol and PS, whereas triglycerides were significantly more than PS (p=0.0004). Bilirubin (0.0001) and bile acids (p=0.0009) were significantly higher than CS (p=0.0001). However, they contained the lowest amounts of sodium, potassium, magnesium, and oxalate. In PS, bilirubin (p=0.0001) was significantly higher than both groups. Bile acid content was significantly higher than CS (p=0.0001) but not significantly more than MS. They showed the highest values of calcium, sodium, potassium, magnesium, and chlorides compared to the other types of stones. High levels of cholesterol in stones and dyslipidemia associated with mixed as well as cholesterol gallstones suggest an etiological association and efforts to reduce dietary fat among the Libyan population may lead to decreased cholesterol and mixed gallstones.Keywords: gallstones; chemical composition; Libya; cholestero

An unusual cause of gastric outlet obstruction during percutaneous endogastric feeding: a case report
<p>Abstract</p> <p>Introduction</p> <p>The differential diagnoses of acute abdomen in children include common and rare pathologies. Within this list, different types of bezoars causing gastrointestinal obstruction have been reported in the literature and different methods of management have been described. The aim of this article is to highlight a rare presentation of lactobezoars following prolonged percutaneous endoscopic gastrostomy feeding and its successful surgical management.</p> <p>Case presentation</p> <p>A 16-year-old boy was admitted to a paediatric ward with abdominal distension and high output from his permanent gastrostomy feeding tube, with drainage of bilious fluids. The clinical, radiological and endoscopical examinations were suggestive of partial duodenal obstruction with multiple bezoars in the stomach and duodenum. Gastrojejunostomy was performed after the removal of 14 bezoars. The child had an uneventful postoperative course and was discharged on the sixth postoperative day in a stable condition.</p> <p>Conclusion</p> <p>Lactobezoars should be included in the differential diagnosis of acute abdominal pain in patients with percutaneous endogastric feeding. Endoscopy is important in making the diagnosis of this surgical condition of the upper gastrointestinal tract in a child.</p
Application of the Savitzky-Golay Filter to Land Cover Classification Using Temporal MODIS Vegetation Indices
In this study, the Savitzky-Golay filter was applied to smooth observed unnatural variations in the temporal profiles of the Normalized Difference Vegetation Index (NDVI} and the Enhanced Vegetation Index {EVI} time series from the MODerate Resolution Imaging Spectroradiometer (MODIS}. We computed two sets of land cover classifications based 011 the NDVI and EVI time series before and after applying the Savitzky-Golay filter. The resulting classification from the filtered versions of the vegetation indices showed a substantial improvement in accuracy when compared to the classifications from the unfiltered versions. The classification by the EVIsg had the highest K (0.72} for all classes compared to those of the EVI (0.67}, NDVI (0.63}, and NDV/sg (0.62). Therefore, we conclude that the EVIsg is best suited for land cover classification compared to the other data sets in this study
Seismic risk in the city of Al Hoceima (north of Morocco) using the vulnerability index method, applied in Risk-UE project

It is demonstrated by the large seismic episodes reported in seismic catalogs and research studies. However, seismic risk is relatively high due to vulnerable buildings that are either old or don’t respect seismic standards. Our aim is to present a study about seismic risk and seismic scenarios for the city of Al Hoceima. The seismic vulnerability of the existing residential buildings was evaluated using the vulnerability index method (Risk-UE). It was chosen to be adapted and applied to the Moroccan constructions for its practicality and simple methodology. A visual inspection of 1102 buildings was carried out to assess the vulnerability factors. As for seismic hazard, it was evaluated in terms of macroseismic intensity for two scenarios (a deterministic and probabilistic scenario). The maps of seismic risk are represented by direct damage on buildings, damage to population and economic cost. According to the results, the main vulnerability index of the city is equal to 0.49 and the seismic risk is estimated as Slight (main damage grade equal to 0.9 for the deterministic scenario and 0.7 for the probabilistic scenario). However, Moderate to heavy damage is expected in areas located in the newer extensions, in both the east and west of the city. Important economic losses and damage to the population are expected in these areas as well. The maps elaborated can be a potential guide to the decision making in the field of seismic risk prevention and mitigation strategies in Al Hoceima.Peer ReviewedPostprint (author's final draft
Religiosity and condom use with casual sex partners in Ghana
Level of religiosity is an indicator of the degree of involvement of people in religious beliefs/activities and a measure of attitudes to sexual-related activities, such as casual sex or using condoms to avert unplanned pregnancy or sexually transmitted infections. This paper uses nationally representative cross-sectional data collected in 2011/2012 to examine the relationship between religiosity and the likelihood of engaging in casual sex and condom use in Ghana. A sample of 4,168 males and females was used to assess sexual activities and condom use with casual sexual partner using binary logistic regression analysis. The study was based on the Reference Group Theory which suggests that religious teachings often dictate the sexual behaviors/attitudes of individuals and was informed by the fact that most conventional religions discourage pre-marital sex and adultery. The study examined how religiosity expressed by individuals impacts casual sex, which is considered a form of adultery. An attempt was made to discover if religiosity impacts condom use as a form of protection against HIV and other sexually transmitted infections and unplanned pregnancy. The results indicate that persons who attend religious meetings more than once a week are less likely to have a casual sexual partner compared to others who attend occasionally or less frequently. No differences between groups with lower level of religiosity are apparent. The level of religiosity did not affect condom use among those who have sex, when other variables are controlled for. Differences in age, sex, level of education, rural/urban residency and marital status are statistically related to condom use with a casual sex partner. The paper concludes that condom use with a casual sex partner may not largely depend on how religious or otherwise one may be, but rather on the individual’s risk perception based on a person’s socioeconomic status, particularly with respect to a person’s level of education
